Initial consultation
In the course of the initial consultation the various treatment processes are explained in detail and you have the opportunity to address any of your concerns. Please allow about 1 hour for this consultation. Because of the amount of time involved prior registration is required.
It is possible, but not necessarily a prerequisite to undergo pre-exams in advance and bring the results to the initial consultation. Useful in this context are, for example:
- basal hormone levels in women
(blood sample taken on day 2 to 5 of the cycle, TSH, E2, FSH, LH, prolactin) - spermiogram in men
In any case, we advise you to bring the following to your first consultation:
- all documents relating to any pretreatment,
- all documents relating to other health problems,
- possibly even older lab results.
We also recommend that before (and after) your initial consultation you draw up a list of the questions that are of greatest concern to you. It is important to us you ask all the questions you have on your mind. If you think that there is something you did not fully understand, we encourage you to please ask again.
Making an appointment
Appointments for an initial consultation are best made by telephone Monday through Friday, 7:30 am to 4:00 pm.
Costs
The costs for the initial consultation are €150.
Providing information & results
Please send us the completed PDF form and copies of any existing test results by post at least one week before your appointment. Please note that, for data security reasons, the use of email or fax is not suitable for transmitting health-related data.
Please complete the PDF form in full and legibly. All fields marked with * are mandatory fields. We need this information so that we have a record of your data at our Center before your appointment; this enables us to begin your initial consultation on time. Thank you.
You can also complete and submit the online form with basic information instead of sending the PDF form via mail. In this event we would ask you to bring the completed PDF form with all information directly to your first consultation.
Once the online form has been sent, the data entered by you appears on the screen once again for you to print or download as a PDF and save for your records.
